HCPTSA Hearing Aid Fund
HEARING AID FUND
Payment limits are set at $26 per ear mold and $56 for hearing aid repair per student. This includes a 6-month warranty. Amended June 8, 2006.
Referrals are received and processed in the following manner:
1. Audiologist will determine if student is eligible for assistance.
2. Audiologist will inform the parents of the total cost and what portion the Hearing Aid Fund will cover. The balance will be the responsibility of the parents.
3. Initially, the entire cost of the molds and/or repairs will be submitted to the Highline Council Treasurer to be paid through the Hearing Aid Fund.
4. If possible the parents can reimburse Highline Council for their portion of the molds/and/or repairs. As parents pay their portion these proceeds will be submitted directly to the Hearing Aid Fund.
5. The Audiologist will retain a dated copy of the letter submitted to the parents and record the date when and how much the parents paid. Copies of all paperwork will be kept in the Audiologist’s office for ease in auditing.
6. Bills for payment should be sent to the Highline Council Treasurer at least every three months.
